Emergency, Medical and Parking Forms
This year the following information will be collected electronically via 3 forms below (instructions are also below). Feel free to contact Anne Hurrle, at ahurrle@bishopchatard.org or 251-1451, Ext. 2248 with questions. ** The medical card and the emergency contact card are required for each student.**
Form Instructions
- Open the form via a link above. Click in the form with your mouse to type into an area.
- To move quickly between fillable blanks, click your 'tab' button.

When finished, save your document, giving it the following title:
Student's Last Name, Student's First Name - Name of the Form (either Emergency Card, Medical Card or Parking Registration)
For example: Hurrle, Anne - Medical Card
- Attach all the forms in one email and mail to forms@bishopchatard.org

If you have more than one BCHS student, please fill out a form for each student.
2011-2012 Free/Reduced Lunch and Textbook Assistance
Any parents who believe their family may qualify for the Free/Reduced Lunch Program and other benefits such as textbook assistance may apply for this program anytime throughout the school year.
To apply,
print the application at this link, complete it, and return it to Meg Horcher, Bishop Chatard High School 5885 Crittenden Ave, Indpls, IN 46220.

Orientation and School Schedule, Aug. 8-12

Mon, Aug. 8: Staff Day of Reflection - School offices will be closed

Tues, Aug. 9: Teacher Work Day

Wed, Aug. 10: Upperclassmen Orientation: Students need to be in the main gymnasium by 8:00 A.M. Students should be in their school uniforms as we will be taking pictures for the yearbook and identification cards. We will have Mass at 11:15 A.M. at Christ the King Church and lunch will be provided for all students following Mass. Students will be dismissed for the day at 2:30 P.M.

Thurs, Aug. 11: Freshmen Orientation: Students need to be dropped off at door 10 (athletic entrance in the rear on the north east end of the building) between 8:10 and 8:30 A.M. Students should be in their school uniforms as we will be taking pictures for the yearbook and identification cards. The day will conclude with Mass at Christ the King Church at 4:00 P.M. and then dinner in the BCHS cafeteria following Mass. Parents and siblings are invited to attend the Mass and dinner.

Fri, Aug. 12: First full student day: Students will visit all classes and their Student Resource Block (SRT) on their first full day. The day will begin at 7:35 A.M. and end at 2:05 P.M.
Parents Back-To-School Night: Aug.16
Our back to school night will be on Tuesday, August 16, 2011. Check-in and pick up of student schedules begins at 6 P.M. in the cafeteria. Your first period (according to your child's schedule) will begin promptly at 6:30 P.M. There will not be a general session this year due to the replacement of the Varsity gym floor. We ask that you please not park in the Christ the King parking lot.
