Sports physicals for the 2008-09 school year will again be offered at Bishop Chatard on Monday, June 2nd from 6:00 P.M.-8:00 P.M. in the school building. The physicals are given by St. Vincent Sports Medicine under the direction of Jon Grant, LAT, ATC & Jason Morgan, LAT, ATC. The cost of the physicals will be $20.00 with $10.00 coming back to BCHS Athletics. Only checks and cash will be accepted. Students with the last names beginning with A thru M should arrive between 6:00 and 7:00 P.M. and those with last names beginning with N thru Z should arrive between 7:00-8:00 P.M.

FOR PHYSICAL NIGHT: Please download the 5 necessary forms below, and have them completed in their entirety when you arrive. Forms will be available the night of physicals, but those that have the forms completed will be seen first.

PHYSICALS MUST BE GIVEN AFTER MAY 1ST IN ORDER TO BE CONSIDERED FOR THE 2008-09 SCHOOL YEAR. .
