Sports Physical/Permission Packets need to be current to participate in Winter sports practices/tryouts etc. Physicals (IHSAA Physical Form) must be dated May 1, 2008 or after to be valid for this school year. Other documents in packet must also be completed, which include Emergency Info, Consent for Athletic Training Services (HIPPA Form), Code of Conduct. Finally Travel Authorization must be completed by every participant; which allows players to be driven by teammates or themselves. Physical packets can be picked up the Athletic Office or downloaded on-line via the links below. If you have any questions, please contact the Athletic office.
