Physicals must be given AFTER APRIL 1, 2011 to be valid for the 2011/2012 school year.

Sports Physicals Night at BCHS: May 31, 2011

Sports physicals for the 2011-12 school year will be held at Bishop Chatard on Tuesday, May 31, 2011 from 6:00PM-8:00PM in the varsity gym. The physicals are given by St. Vincent Performance under the direction of Jon Grant, LAT, ATC & Jason Morgan, LAT, ATC. Students with the last names beginning with A thru M should arrive between 6:00 and 7:00 P.M. and those with last names beginning with N thru Z should arrive between 7:00-8:00 P.M. (You do not need to be a BCHS student to attend. Grade school athletes - bring your CYO form and be ready for next year.)

• Cost: $20, with $10 benefiting BCHS Athletics
• Cash or checks (payable to Bishop Chatard H.S.) only

We strongly encourage all athletes to attend the sports physicals night. This physical is valid for the entire 2011/2012 school year.

FOR PHYSICAL NIGHT: Please download the 4 necessary forms below, and have them completed in their entirety (with signatures) when you arrive. Forms will be available the night of physicals, but those that have the forms completed will be seen first. FORMS MUST BE COMPLETE BEFORE YOU GET IN LINE.

TICKET TO PLAY
Ticket to Play is an eligibilty pass that will be issued for each student after all 4 Physical forms (see below) are received. It is verification that the student is an eligible athlete, with all paperwork on file in the Athletic Office. Every student-athlete needs to present a personal "Ticket To Play" to the head coach of each sport they participate in before they can begin any team involvement-- summer or seasonal conditioning, practice, etc. Tickets for the 2011/12 school year can be picked up in the Athletic Office after June 6, 2011.

ALL PAPERWORK MUST be completed and signed as noted on the forms.
(The forms can also be picked up at the Athletic Office at any time.)
